Help accelerate the claims process by providing beneficiary SSN and DOB during application
When submitting a life insurance application with Protective, it is best practice to provide the beneficiary’s Social Security Number (SSN) and date of birth (DOB). Obtaining the beneficiary SSN and DOB during application results in faster claim servicing and verification for our beneficiary clients, and helps you fulfill Know Your Customer and anti-money laundering (AML) requirements.

Important news for Banner paper app users
Effective August 1, Banner will no longer accept the traditional paper life insurance application. ALL applications must be submitted with either the new fillable PDF application or the digital application.
Important highlights:
- No more paper apps! Download the new fillable PDF application here.
- No more exam or APS ordering! Banner orders ALL underwriting requirements.
- No more checks! Banner will collect payment details electronically. Physical checks will not be accepted.
- No more playing telephone! Banner will reach out to clients direct throughout the underwriting process via email. Please ensure the client’s email address is typed in correctly and tell them to open and respond when needed.

- To simplify the application process for all involved
